Structured Medication Review

At Moseley Avenue Surgery, we offer Structured Medication Reviews to help ensure your medication is safe, effective, and working well for you.

These reviews are carried out with our Clinical Pharmacist, working as part of the wider Primary Care Network alongside your GP practice.

 

What Is a Structured Medication Review?

A Structured Medication Review is a confidential discussion about your medicines and overall treatment plan. The review usually lasts around 30 minutes and gives you the opportunity to:

  • Discuss how your medication is working
  • Review any side effects or concerns
  • Ask questions about your treatment
  • Ensure you are receiving the most appropriate medication for your needs

If any changes to your medication are recommended, these will always be discussed and agreed with you before any adjustments are made.

 

Preparing for Your Appointment

Before your review, please ensure you have access to all of your medications, including:

  • Prescribed medicines
  • Inhalers, creams, liquids, or ointments
  • Vitamins, supplements, or herbal remedies
  • Any medicines purchased over the counter
  • Medicines you no longer take

You may also find it helpful to make a note of any questions or concerns you would like to discuss during the appointment.

 

Important Information

  • A member of the practice team may contact you by telephone or text message to arrange your appointment
  • The Clinical Pharmacist may call from an unknown number
  • Some medications may require ongoing monitoring, such as blood tests or blood pressure checks
  • A summary of your review will be added to your medical record and shared with your usual GP where appropriate
