Access to Medical Records

If you would like to access your medical records, you can do this by signing up for the NHS App. You can register for this here.

Please note that to register for online access, you will need to provide ID: 

Appropriate ID adults - Passport/Driving Licence/Student card
Appropriate ID children - birth certificate with parent/s details or UC/benefits claim whereby children are named on the claim.

If you do not agree with an entry/ entry’s in your medical record or believe a mistake has been made in the record, you do have a right to challenge this and is known as “ The right to rectification”. For information about this, please click here.

It will raise questions that you may not have considered to help you decide whether to access your record in this way. Please answer all the questions as best as you can.

 

We can grant access to online medical records, provided you upload your photo ID.
