Diabetic Reviews
To help us complete your review effectively, please:
- Print your blood test form at home or collect a copy from reception and arrange your blood tests
- Bring a fresh first urine sample on the day of your foot-check appointment (white-top sample bottles are available from reception)
COPD & Asthma Reviews
Please bring all of your current inhalers to your appointment so we can review your treatment and ensure you are using them correctly.
Travel Immunisations
Before booking a travel appointment, patients are asked to complete a travel questionnaire (Florey form). This helps our nursing team prepare in advance and ensures your appointment is as efficient and informative as possible.
Baby Immunisation Clinics
For all childhood immunisation appointments, please remember to bring your child’s Red Book.
