Supporting NHS Planning & Research

Coventry & Warwickshire Integrated Care Record
Moseley Avenue Surgery works closely with health and social care organisations across Coventry & Warwickshire to support the Integrated Care Record. This enables authorised healthcare professionals involved in your care to securely access relevant medical information, helping them make informed decisions and provide safer, more coordinated care.
Access is limited to professionals directly involved in your treatment and care.

Sharing Your Information
Advances in healthcare technology now allow GP records to be securely shared electronically with other NHS and health and social care providers when appropriate and with your consent. This helps ensure you receive the right care, from the right service, at the right time.

- Dentists, opticians, and pharmacists
- Community and voluntary healthcare providers
- Ambulance services
- Primary Care Networks and NHS organisations
- Social care services and local authorities
You have the right to object to or withdraw consent for data sharing at any time. Wherever possible, your consent will be requested before your records are accessed, and your care team will always explain if limiting information sharing could affect the care available to you.

The National Data Opt-Out (NDOO) service lets people decide whether their confidential patient information can be used for research and planning.
