Please send us an "admin query" asking "pill form please" and a member of our admin team will send you the form, straight to your mobile or pc/laptop, using your email address. So please ensure we have your most, up-to-date details and please check your emails regularly.

We are a paper light surgery and avoid printing paper, where it isn't necessary to do so.
You may request a repeat prescription for your pill/patch if you are taking a pill/patch that has been prescribed by our practice and there are not any reasons why we should not continue to prescribe it.
Please ensure you fill out all relevant parts of this form and insert 3 blood pressure readings, if you do not, then we will be unable to process the form.
3 blood pressure readings are now required, regardless of what contraceptive pill you are on.
Risks***
Although the overall risk of having a bloodclot as a result of taking the combined pill is small, for some women it may be a serious risk. This risk is present for all women who take the combined pill (risks include bloodclot in leg (DVT) Clots in lung (PE), stroke or heart attack. The risk is increased if you smoke, travel on long-haul flights (for more than 3 hours) trek at an altitude greater than 2500m, have recently had an operation, or are bed-bound.
